MMS Safety Observation Card

To access the Safety Observation Cards, Click on the “Safety First” Image or the “Access Safety Observation Form” Link.

You may be prompted to provide your MMS Office 365 username and password (see instructions below).

Your username is your MMS email address

Your password is the password you use to access OUTLOOK.
